Summary:
During a takeoff on Runway 03 with a reported surface wind of 320ยบ/ 12 kt, the aircraft started swinging to the left as the tailwheel came off the ground. The pilot corrected for the yaw but was unable to control the resultant lateral oscillation. The aircraft went off the right side of the runway and the left gear collapsed.
